Share Your Story of Success!

Greetings Nebraska partners!

As a partner agency, participating in the Nebraska State Health Improvement Plan (SHIP), we invite you to share your story of success. We are looking to highlight programs and services in the central and western part of Nebraska that promote socio-emotional-mental well-being, support suicide prevention or address depression, and are positively influencing individuals, families, and communities with successful outcomes. We hope to expand the awareness of activities and organizations that contribute to this important cause, and increase the dialogue about the role of public health, early childhood, and family and community supports.

Please take this quick survey (5-10 minutes) to highlight your organization and provide a success story about one of your programs or services.
https://www.surveymonkey.com/r/NFFFS7W   Please complete by February 28, 2019.

Your success story will be highlighted in a final report that is designed merely to recognize the great work many organizations across Nebraska already do to serve their communities.
The SHIP holds no ownership over your story, but is honored to disseminate the report of success stories to interested partners statewide. We want to celebrate your success, and reflect the magnitude of effort that committed people contribute to this great state.

Thank you for sharing your story of success!
